Wellness Network
JFS’ Wellness Network works to improve health equity by reducing barriers to high quality, coordinated health care, improving health literacy, and promoting participation in preventative care. Network clients are those who face the greatest barriers to accessing care, including individuals with serious mental illness (SMI) and people living below the poverty line—putting them at higher risk of preventable hospitalization and emergency care. Many are also managing one or more chronic conditions, such as diabetes, COPD, and anxiety/depression.
The Network uses evidence-based practices to improve health outcomes and reduce emergency and acute care for chronic conditions. The program also partners with medical and social service partners to connect clients to a full continuum of primary, specialty, and preventative care.
Our aim is to support people to live longer, healthier, and happier lives. Tailored for adults in Atlantic County, the JFS Wellness Network offers services that can be experienced either one-on-one or in group sessions.
The program offers a variety of resources to empower the wellness journey, including support for managing diabetes, guidance from compassionate nurses, mindfulness practices for self-care, tips on cultivating healthy eating and exercise habits, and assistance with preventative healthcare measures. JFS staff help to navigate relationships, understand the latest healthcare recommendations, and ensure connections to important screenings for early detection. Plus, our team assists in scheduling follow-up vaccinations, including those for shingles, pneumonia, COVID-19, RSV, and more.
