Case Manager – Community Support Services (CSS)
Join the JFS team and help individuals with serious mental illness achieve stability, wellness, and autonomy. ICMS Case Manager / Hospital Liaison
The ICMS Case Manager/Hospital Liaison provides compassionate, client-centered services to adults diagnosed with severe mental illness (SMI). This role focuses on building therapeutic relationships, promoting recovery, and ensuring smooth transitions from psychiatric hospitals to the community. As part of a collaborative, interdisciplinary team, the Case Manager supports clients in achieving personal goals, accessing critical resources, and maintaining stability in the community.
The Hospital Liaison component of the role focuses on active engagement with psychiatric units to identify appropriate referrals, support discharge planning, and ensure continuity of care.

Duties & Responsibilities
The supervised delivery and coordination of services to consumers include:

JOB SUMMARY (This position is located in Atlantic County, NJ)
As the case manager, you will meet with youth and their families multiple times per week to offer support, education, and linkage to resources to increase the likelihood of success in completing the HEDS program. You will need to be a good listener and good communicator. In response to individual client / family needs you will work with the youth and their parents/guardians to develop ways to manage behavioral, emotional, relationship and legal challenges. Providers are committed to working with all individuals regardless of race, gender, religious affiliation, cultural beliefs, lifestyle, and disease process or treatment plan.

- Complete intakes with new referrals and their families to include a risk assessment

Job Description:

Complete financial registration and verify insurance for clients
Obtain, update, and manage service authorizations
Review weekly billable services and prepare billing batches

What We’re Looking For
We’re seeking a skilled therapist who brings:
- Experience working with diverse populations across the lifespan
- Strong communication, organization, and problem-solving abilities
- A commitment to ethical practice and quality care
Position Requirements
- Education/Licensure: Master’s degree in Social Work, Counseling, Psychology, or related field. LCSW required
- Experience: 2+ years in a counseling, mental health, or social work setting
- Valid driver’s license required

JFS Food Pantry Volunteer(s)
JFS is looking for a volunteer or a group of volunteers needed to take on a weekly role in our food pantry. The position would entail inventory, ordering, completing monthly food bank report, organizing our pantry client delivery list (both new and existing), tracking donations, talking with clients and volunteers to coordinate.
