Older Adult

Jewish Family Service provides outreach, in-home psychotherapy, and geriatric assessments to older adults with the goal of enhancing their quality of life and helping them to maintain an independent lifestyle. See current fee schedule below.

Programs include:

Care Management Services
Jewish Family Service’s Fee-for-Service Senior Management Services provides comprehensive assessments and services to aging individuals who are unable to look after themselves without assistance. Our professionally trained staff work with you to ensure that social, emotional, nutritional and medical issues are addressed with compassion. Referrals most often come from relatives at a distance who rely on JFS to substitute for the family, providing support and options to nursing home placement.

Care Management Service: Initial Assessment $150.00, then $60.00 per hour

Family Guidance/Long-Term Planning
A skilled clinician meets with relatives to determine the needs of an aging family member, then prepares a plan for intervention, which may include referrals to area resources. There is a fee for this service.

Counseling
Services offered are available in the JFS office or the home if necessary. Fees are usually covered by Medicare and other third-party insurance companies.

Harry L. Katz Medical Advocacy Program
JFS offers medical advocacy, care management and nursing consultation for individuals with acute health issues. Nutritional, medication, and clinical oversight are provided.

Klinghoffer Nursing & Nutrition Program provides older adults a comprehensive support system in order to develop and maintain a healthy and balanced diet while ensuring good health. The program seeks to improve the well-being of older adults with limited income and multiple medical conditions by helping them to obtain the good food choices at optimal quantities. Through a professional nurse’s assessment, dietary plan and care, seniors are able reduce complications and decrease hospitalizations related to diet and there medical needs.

Senior Healthcare Connections
In collaboration with Beron-JOAS, JFS provides a one-time nursing assessment for individuals with complicated medical conditions, creates a health management plan, and provides ongoing monitoring. This is funded by the The Foundation of Jewish Federation of Atlantic & Cape May Counties.

Alzheimer’s Services
JFS provides counseling and care management services in conjunction with The Richard Simon Center for Alzheimer’s at The Seashore Gardens Living Center.

Diabetes Day by Day
Diabetes Day by Day is a care management and outreach program funded in part by United Way of Atlantic County. The program serves seniors who are experiencing trouble with their diabetes or have been recently diagnosed. It provides both diabetes education and care management services.

Kosher Home-Delivered Meals
The Kosher Meals-on-Wheels program operates jointly with Beron-JOAS. The program provides Kosher home-delivered meals to elderly individuals residing throughout Absecon Island.

Kosher Meals on Wheels: $7.50 per frozen meal

Senior Transportation Options Program
Senior Transportation Options Program or (STOP) is a cost sharing ride program. We provide transportation to doctor appointments and other healthcare visits at a nominal cost. This service is available to existing agency clients only. We gratefully acknowledge financial support for this program by Rite Aid Pharmacy and Ocean City Bank.

Holocaust Survivor Services
As part of a state contract with the Claims Conference on German Reparations, JFS provides a subsidy for homecare and social service assistance to Holocaust survivors and to individuals who resided in occupied countries during the Holocaust. Monthly luncheons and a variety of social activities are planned throughout the year.

Shalom House Services
In partnership with Jewish Older Adult Services (JOAS) and under contract to Shalom House, JFS provides homecare and housekeeping services to residents of Shalom House. A full-time social worker also provides assessment, information, referral and social service coordination to the building’s elderly residents.

Fees for programs and services

  • Volunteer Services: No cost, except for the $60.00 per hour charge for a case manager to visit on an as-needed basis.
