Community Support Service Worker

SUMMARY

Your core competencies in soft skills include: communication, problem solving, initiative and negotiation You possess the highest levels of accuracy, initiative (in solving problems and organization), tact and professionalism. Your specific job tasks include supporting clients in the community/their home to complete interventions in accordance with their individual service plan (IRP) which may involve support of ADLs including household chores, shopping, budgeting, accessing transportation as well as providing social support opportunities.

REQUIREMENTS

  • Education – Associate Degree in helping profession plus minimum of 2 years working in community based behavioral health setting, Certified Psychiatric Rehabilitation Practitioner (CPRP) may be substituted for one years’ experience or High school diploma/equivalent and minimum of 3 years working in community based behavioral health, CPRP may be substituted for one year’s experience
  • Experience – Work or volunteer experience in a related area, preferably with a similar population

RESPONSIBILITIES

  • Communicate with consumers to understand their IRP goals
  • Coach consumers to complete interventions as defined
  • Guide clients in learning new skills tasks
  • Develop necessary contacts/resources to support consumers
  • Document activities in an accurate and timely manner
  • Maintain regular, professional communication with Consumers, Program Supervisor and Program Staff
