Case Manager - ICMS
At JFS, Community-based ICMS Case Managers bring together resources and services in response to individual consumer needs assisting them in achieving wellness and autonomy. ICMS Case Managers are active members of an interdisciplinary team; your skills of engagement are needed to complete comprehensive assessments, develop service plans, participate in reaching program goals, and assuring ongoing quality improvement initiatives. Case Managers are committed to working with all consumers regardless of race, gender, religious affiliation, cultural beliefs, and lifestyle.
Skills and Responsibilities:
The delivery and coordination of services to consumers includes:
- Individual, group and family intervention
- Outreach case management and support
- Referral and advocacy for linkage to essential services based on consumer choice
- Connection to social, spiritual and other community resources as expressed by the consumer
- Ensuring documentation and productivity per agency standards
Qualifications
Bachelor’s Degree or Masters Degree in Social Work or related Human Service (Psychology, Behavioral Health, Counseling, Psychiatric Rehabilitation). One year of MH setting preferred for case managers with Bachelor’s Degree
