Programs & Services

Mental Health Services
Supportive outreach services are provided to individuals with psychiatric needs. Counseling, group activities, self-help skills, advocacy, medical referrals, psychiatric evaluations and treatment are offered. Specific programs include:
Integrated Case Management Services
(ICMS) Integrated Case Management Program case managers provide a minimum of eighteen months of aftercare services to all individuals leaving the State Psychiatric Hospital and to some clients leaving the Atlantic City Medical Center. The program counselors assist consumers in successfully reintegrating into the community.
Project New Chance
(PNC) Project New Chance primarily serves elderly individuals who receive outreach support to maintain their physical and emotional stability.
The PATH Program
The PATH Program is funded by a contract with the New Jersey Division of Mental Health Services. It provides street outreach throughout Atlantic County to individuals in homeless situations with serious psychiatric disorders. JFS staff also work out of the Atlantic City Rescue Mission and Sister Jean’s Kitchen.
Community Access Programs
Atlantic County Diversion Collaborative This program provides services designed to prevent or substantially shorten the incarceration of mentally ill individuals arrested for non-violent, victimless offenses. Jewish Family Service is deploying five staff members to coordinate and provide case management help. The agency's partners, AtlantiCare Behavioral health and the Family Service Association, will provide recovery focused mental health interventions to the one hundred fifty people served each year.
Community Re-Entry
Jewish Family Service is providing help to inmates with a mental illness who are completing their sentence at the Atlantic County Criminal Justice Facility re-integrate into the community by connecting them to mental health and housing services.
Supportive Housing Services JFS provides support to individuals with psychiatric difficulties who live in independently in Atlantic County. The program serves individuals needing outreach support to maintain their household and develop integration and natural support systems in the community.
Behavioral Health Collaborative
In partnership with AtlantiCare Behavioral Health and the Atlantic City Rescue Mission, JFS provides case management services to a five-bed transitional residential unit that helps program participants in psychiatric crisis be diverted from psychiatric hospitalization. The case manager supports individuals during their tenure in the unit and links to needed community services. A community linkage/re-entry group is also provided by the case manager as part of the program’s partial care programming.
TANF/GA Case Management
Jewish Family Service has case managers
located at the County Building in Atlantic City who conduct
assessments to identify mental health difficulties and provide
case management support. They connect consumers to either long-term
disability or to the workforce, through county-sponsored employment
or training services. The program combines assessment, crisis
intervention and case management, and deals with consumers'
multiple problems. |
