Wachovia Bank Supports JFS Health Care Advocacy Program

Jewish Family Service has received funding from Wachovia Bank to support the Health Care Advocacy Program which assists older adults with supplies, food and medicine following a hospitalization.  This program is intended to alleviate the serious challenges encountered by older adults who are isolated, forced to recuperate without family in this area. They often lack proper nutrition at home and are without means to obtain their prescribed medications or other supplies. Wachovia funds will provide assistance for items not covered by insurance to help individuals to stay in the community.

JFS’s Health Care Advocacy Program will continue to provide interventions that assist older adults to overcome the fragmentation in the health care system that creates medical errors and failure in communication among physicians and practitioners participating in a patient’s care. The program provides outreach to two hundred individuals each year to ensure that they receive an optimal level of care following an inpatient hospitalization when they lack the wherewithal to meet their medical needs. It has provided potentially life-saving interventions to individuals whose medical care is not coordinated and has markedly enhanced the quality of life of program participants.
